Job Summary
To support the Health Centre management in providing Psychosocial services including HIV case management to patients and their families based at the ART clinic.
Overall Responsibilities
Train and mentor case management officers and health facility focal persons Identify and enroll eligible clients into the HIV case management model based on HCM criteria.
Support projects supported health facilities to implement the YAPS and other similar model to improve HIV prevention and treatment outcomes among Adolescents and Young People (AYP)
Provide technical guidance to health facility teams in use of the HIV case management platform for expanding Index HIV testing, TB testing and TPT initiation, and uptake of MMD and community DSD models.
Support health facilities to conduct and coordinate health education sessions to improve retention in HIV care and adherence to ART including enrolling non-adherent children and adults into the HCM for individualized adherences support
Liaise with the TB/HIV technical officer to support health facilities to implementing Advanced HIV disease (AHD) screening, diagnosis, and management guidelines.
Provide Case Management officers in convening of multi-disciplinary health facility teams to discuss and develop individualized care plans for problem ART clients, and monitor their implementation
Provide technical guidance to district mentors and coaches in the roll out and continued use of care Audit trackers for children and adolescents, the virally non-suppressed, TB cases and HIV positive Pregnant and Breast-Feeding Women (PBFW).
Mentor health facility teams to provide psychosocial assessment of HIV patients and their families and provide support or refer to community service providers for complementary support as appropriate.
Support Health facility teams to implement and document QI projects on HIV case management.
Mentor Case Management Officers and other Health Facility staff on timely documentation and updating of patient information in the appropriate registers and HIV cards under HIV case management.
Participate in weekly/ monthly/ quarterly data reviews meetings and give feedback to clinical staff on patient’s progress under case management.
Mentor case management officers and other health facility staff on graduating eligible patients from HCM to appropriate DSDM.
Required Qualification, Experience and Competencies:
A bachelor’s degree in social work, Social Sciences, Development studies, Nursing, Psychology, counselling or a related discipline.
Additional training in counselling, nursing or any clinical discipline will be of an added advantage.
At least two years of experience in implementing community targeted HIV and MNH interventions.
Have at least two (3) years’ experience in engaging District and community stakeholders to set up and support multidisciplinary case management teams
