Multi Community Based Development Initiative Field Assistant – Facility Jobs in Uganda

Job Purpose:

The Field Assistant-Facility is responsible for the coordination of facility activities, session roll out and reporting at health facility level. The Field Assistant-Facility will closely work with other structures at both community and facility levels and will be responsible for facility level mobilization, coordination, oversight, data collection, review and consolidation of facility level reports across Sub counties.

Key duties, responsibilities and deliverables

Coordination:

• Periodically visit households of non-suppressors and support non-suppressing children to ensure they suppress.

• Develop emergency response plans for CLHIVs as non-suppressors.

• Ensure attachment of all non -suppressed C/ALHIV to respective Community Health Workers for close monitoring and DOTs implementation

• Ensure proper documentation of all facility interventions and routine updates of the OVC files at facility level.

• Liaise with program specialist to organize facility and non -suppressor’s case conference meetings including switch meetings

• In liaison and coordination with the community team, ensure timely follow ups of non- suppressors and missed appointments

• Coordinate the works of Case Care Workers at all times

• Participate in Sub County planning and coordination activities (SCWC meetings)

• Participate in performance review meetings (Districts and Sub County) and events.

• Network and collaborate with relevant partners at the Sub County and facility levels for provision of comprehensive services to OVC

• Support Community Health Workers directly observe treatment for non-suppressed

• Work with Counselors to follow up missed appointments, lost to follow ups and non- suppressed either physically or through phone calls and have the follow up efforts documented in the respective registers and forms

• Request for emergency fund for critical cases

• Liaise with Counselor or Clinician or ART In-charge to line list Pediatric and adolescent for services including ART refills, IAC and viral load bleeding and index testing

• Work with PSWs to make pre-appointment reminders virtually for clients to avoid missed appointments and document evidence of contact on blue card and appointment book

• Participate in health education talks including sensitization on OVC services targeting children and their caregivers, develop schedules and compile reports detailing what was taught and who participated, best practices and challenges.

• Support periodic data collection and ensure timely submission

• Support referral and linkage of OVCs for services such as education services, apprenticeship, Young positive, Family Support, PSS Groups and ensure that the community facility registers are timely updated.

• Support documentation and updating of OVC files at the Health Facilities and ICARE offices

• Routinely review the audit tools to identify children that are due

for services including;

enlisting children that are due for VL, due for appointment, those with low and high Viremia for VL, missed appointments, missed VL etc. and ensure timely updates after service provision.

• Act as a focal point person for bi-directional referrals, support CCWs to guide clients where to get services and ensure documentation of the facility-community register and afterwards file them.

• Profile OVCs (C/ALHIVs, SVAC, HEI with non-suppressed mothers, children of female sex workers and adolescent Mothers) not enrolled and work with other Field Assistants to have them enrolled. Share unique identifiers of OVCs enrolled with the Counselor or Data Person for update in the audit tool and label OVC files including updating the PSS 8 and 11 on the blue card.

• Participate in the routine HIV patient screening activities for TB, HTS, STIs including entry of data and tracking of HIV testing services

Supervision:

• Supervise Case Care Workers and Roving Case Care Workers to ensure effective and quality program delivery

• Periodically appraise the performance of the team under your supervision.

• Mentor, coach and train the team under your supervision.

• In consultation with Program Specialists, represent ICARE in the Facility and sub county level meetings and other events.

• Conduct community dialogues and outreaches closely with other structures

• Roll out respective sessions in line with your speciality (VSLA, BIM and Journeys)

• Participate in community activities including community drug refills, viral load bleeding, IAC and HTS and update client contact forms, family trackers.

• Complete a monthly cascade report for sharing with supervisor and collaborate with the Monitoring and Evaluation team to ensure data collected is accurate and valid.

Minimum Qualifications and experience:

• Must possess a diploma in any Social Sciences discipline or At least 5years experience implementing project activities at community and facility level.

• Good communication skills both written and verbal

• Possess basic knowledge in HIV/AIDS programming

• Experience working with OVCs

• Good skills in Networking and collaboration
